Please find attached the Registration Form and Health Questionnaire.

Kindly complete both forms on a computer (please do not fill them out by hand), print them, and submit the signed copies to the school office by July 10^th Wednesday.

In addition to the completed forms, please bring all the required supporting documents listed on the last two pages of the attached document.
