Please find attached the Registration Form and Health Questionnaire.
Kindly complete both forms on a computer (please do not fill them out by hand), print them, and submit the signed copies to the school office by July 10th Wednesday.
In addition to the completed forms, please bring all the required supporting documents listed on the last two pages of the attached document.
